Collections
Payment is due in full for all charges within thirty (30) days from the date of the first statement. If paid in full by that date, there will be no finance charge.
Fees & Payments
Payment is expected at the time of service for all routine eye care, unless prior arrangements have been made. We accept Mastercard, VISA, and Discover as payment on account.
Medicare
All our physicians participate with Medicare. We will electronically file all claims for you. You are responsible for paying the amounts applied to the deductible and the 20 percent co-insurance. If you have a secondary insurance plan, we will also file automatically to them. Refractions and certain other services are not covered by the Medicare Program. You will not receive a statement unless there is a balance after Medicare and your secondary insurance has been processed.
Workman's Compensation
When scheduling your appointment, please let us know that you are here for care due to a workplace illness/accident or auto accident. Failure to do this may result in your claim being filed to the wrong company and denied by your regular health insurance carrier. We reserve the right to contact your employer prior to care for a work-related injury or illness. We will request both health and auto insurance verification for care related to auto accidents.
